Responsibilities/Duties:

• Cooperate with therapy teams and Rehabilitation Department to promote patient well-being.
• Verify that all evaluations and associated documentation are done.
• Establish clear goals and objectives: Establishing clear goals and objectives will help you assess your progress and determine when you have achieved success. Ensure that all staff members understand what the goals are and how they can help achieve them.
• Deliver therapeutic interventions that align with the treatment plan and any accompanying prescriptions.
• Provide interventions that cultivate patients' talents and/or shortcomings.
• Display proficiency in managing clinical workflow by documenting treatment procedures in electronic records systems.
• PDPM (Patient-Driven Payment Model) reimbursement rates are calculated by assigning patients to various payment categories based on specific clinical criteria.
• Tailor therapy treatments and plans to meet the specific Medicaid and Medicare billing criteria.

Qualifications:

• The New Jersey Licensing Board regulates the physical therapy profession and enforces compliance with the state's rules and regulations.
• As a specialist in long-term care, my experience in Skilled Nursing Facilities has allowed me to acquire a wealth of expertise, ensuring that residents receive the highest standard of care.
• Capability in organizational, communication, and interpersonal skills.
