Responsibilities/Duties:
- Partner with therapy teams and Rehabilitation Department to enhance patient recovery.
- Ensure completion of all evaluations and documentation required.
- Implement feedback mechanisms: Feedback mechanisms such as surveys or feedback forms can help you identify areas that need improvement in communication and assessment processes. Use this feedback to continuously improve these processes.
- Implement the recommended therapy interventions and treatments as specified in the treatment plan and prescription.
- Offer therapies that enhance patients' proficiencies and/or limitations.
- Competently handle workflow management by keeping track of patient information and symptoms in electronic records systems.
- The PDPM (Patient-Driven Payment Model) model seeks to reduce fraudulent billing practices and provide equitable, patient-centered care.
- Adjust therapy treatment approaches and plans to align them with the Medicaid and Medicare billing processes.
Qualifications:
- In New Jersey, Physical Therapy Aides must have completed an approved physical therapy program and successfully pass the national physical therapy examination to become licensed.
- With a focus on long-term care, my experience in Skilled Nursing Facilities has allowed me to gain a deep understanding of the individual needs of residents, providing them with the specialized care they need.
- Outstanding communication, organizational, and interpersonal skills.
- Healthcare providers must be adept at managing patients with different personalities and abilities to provide optimal care.
